Monitor ownership classes (managed / self-service) with scoped alert routing

Every monitor now carries an ownership class. Self-service monitors are fully customer-managed and alert the customer's recipients only. Organization-level integrations are no longer paged for them. Existing monitors stay managed with unchanged behavior.
